• One academic reference from a nurse with a master's or doctoral degree; one from a nurse in a leadership position; one from an individual to whom you directly reported in the last six years
• Transcript
• Resume or CV
• Statement of interest
• Registration with the College of Nurses of Ontario or eligibility for registration (or equivalent in home province). Photocopy will be accepted.

Other Requirements

• Three satisfactory letters of reference (e.g., minimum rating of ‘good’ in all letters), including:
  1. an academic reference from a faculty member who taught the applicant at the graduate level;
  2. a reference from a direct workplace supervisor of the applicant within the past 5 years; and
  3. a nurse in a leadership position who can evaluate the applicant’s actual and potential contributions in the profession of nursing;

• Graduate Supplementary Application: Faculty members' research interests as well as those currently available to supervise PhD students are listed on the website. From that list, initially please identify on the application form 2 or 3 faculty members who are available to supervise PhD students and are doing work that coincides with your own interests.
  • Please note - in order to have your application considered for admission, you must have had an interview/spoken to the faculty member and obtained their agreement in writing to be your supervisor. Complete the PhD Supervisor Applicant Form and upload it to myfile account before the application deadline.
